Print and complete the following documents:
1. Registered Services Agreement
2. Terms & Conditions (do not need to send to OK.gov, for your records only)
Step 2
Include a check for $75.00 for the initial account fee. Your account will be billed annually for $75.00 on your anniversary date.
After documents are completed and signed, mail or fax to:
