The Procedure for Filing and Investigating a Request for Inquiry Concerning a Licensed Psychologist:
The Oklahoma State Board of Examiners of Psychologists has the responsibility for enforcing the Laws and Rules governing psychologists found in the Psychologists Licensing Act (Title 59, O.S., Sections 1351-1376) and in the Rules of the Board. The investigation of a request for inquiry concerning a licensed psychologist may be initiated only when the request for an inquiry is in writing, is signed and notarized, and filed with the Board. The notarized request for inquiry must contain a brief statement setting forth the allegations of fact and naming the licensee or licensees it concerns. The request for inquiry then provides the basis for the Chair or Vice-Chair of the Board to request an informal meeting with the licensee, whose participation is voluntary, and/or to appoint an investigator. Investigators are chosen from a pool of licensed psychologists trained specifically for this purpose. Please note that the Board may provide copies of the request for an inquiry to the psychologist about whom the request for an inquiry is made. The Board shall have the power and duty to suspend, place on probation, require remediation, or revoke any license to practice psychology or to take any other action specified in the rules and regulations whenever the Board shall find by clear and convincing evidence that the psychologist has engaged in unethical or illegal behavior.
Important aspects of this process are outlined on the Request for Inquiry form provided below. To submit an inquiry, please download the Request for Inquiry Form and return the notarized form along with any documentation supporting the allegations to the Board office:
