Motor Carrier Safety Administrative Hearing Request

This request form is intended for motor carrier companies challenging the civil penalties assessed in the Notice of Claim for Out-of-Service violations from a roadside inspection.

This form must be completed and submitted within 25 days of the date the Notice of Claim was sent.
Please use the Notice of Claim sent to the company from the Oklahoma Highway Patrol - Troop S to complete the following section. Only those Out-of-Service violations for which a Civil Penalty has been assessed may be challenged.
NOTE: If you, the motor carrier, are organized as a corporation, LLC, or a partnership, you may not proceed to a hearing unless you are represented by an attorney licensed in Oklahoma. You must be represented by an attorney at the hearing and throughout every stage of the hearing and administrative process.

If submitting by email, documents must be sent as a pdf attachment and should be attached in reply to the confirmation email that includes your assigned case#. DO NOT start a new email thread.
If submitting by postal mail, please include the following:
1. A copy of the hearing request.
2. Images must be clear and printed in color.
3. Any documents submitted must be labeled with the corresponding violation code.
ALL documents and/or images must be printed separately. DO NOT print front and back.
Video can be submitted via email as an attachment. DO NOT send as a link. You may also send a video by postal mail.
