Anyone planning to open a food service or lodging business should note that the Oklahoma State Department of Health (OSDH) requires submission of a Plan Review Application along with a nonrefundable $200 application fee as part of a process to help the applicant comply with licensure regulations.

''This plan review process will allow us to consult with restaurants and hotels at the beginning of the process so we can eliminate costly mistakes in the construction or conversion of the establishment,'' said OSDH Consumer Health Service Chief Vernon Bolz.

An Application for Plan Review (Form No. 824) should be submitted to the county health department in which the establishment is located for new construction of an establishment, conversion of an existing structure for use as a food service establishment, remodel of an existing food service establishment, or for an existing establishment that plans to change its type of operation.

Applicants will be asked to provide a proposed layout or floor plan; the intended menu and anticipated volume of food sold, stored, prepared or served; and proposed equipment types including the manufacturer and model numbers, if available. Applicants should also expect to provide additional information that may be requested by the agency for proper review such as the finish schedule, plumbing, mechanical, as well as construction materials used.

Select ''Retail Foods'' or ''Hotels-Motels'' for county-specific application and submittal instructions.
