The HELP Check Logo Incentive Program has been extended for the 2013 plan year.  This incentive allows eligible members to receive a $200 payment for completing certain requirements.

HELP Check Logo is designed to help members identify their health risks in an effort to improve their health. After members complete the requirements of the program, they will know what they need to do to reduce their risks and improve their health. The program is available to all primary, HealthChoice health plan members age 20 and older who are not enrolled in a HealthChoice Medicare Supplement Plan.

To qualify for the HELP Check Logo incentive, members must have completed the following requirements:

  1. Registered online for the HELP Check Logo program at www.sib.ok.gov or www.healthchoiceok.com
  2. Read all program information.
  3. Complete their free comprehensive preventive service.
