Additional Certification Criteria for Varicose Veins

Additional criteria have been developed and added to the review process for medical necessity of Saphenous Vein Ablation – Laser and Radiofrequency. This updated criteria will be implemented on January 1, 2013.

1. Surgical procedures shall be limited to one surgical session per leg.
2. Additional requests shall be considered after the clinical outcomes of prior treatment(s) have been established and documented. Updated clinical information, including continued symptoms and outcomes of previous procedures, will be required with each new subsequent request. Color pictures will be required when requesting subsequent procedures.
