Child Outcome Summary Forms are used by providers serving children 0-3 years of age in Sooner Start Early Intervention, and providers serving children 3-6 years of age in local education agencies. These forms must be completed twice for children who have received services for 6 months or more. Once at the child's entry into the program and again at the child's exit from the program (age 6). Best practice is to use two or more sources of information when completing these forms. Keep in mind; these forms and the numerical scale are based on typical child development.
