Peer Recovery Support Specialist Forms

 

Peer Recovery Support Specialist (PRSS) On-Line Application & Associated Forms 

  • Access to the new online system (click here) must create a user id and password first before you can access the new on-line application process.
  • New PRSS On-Line Application & Associated Forms – Please make sure your information is correct, but just incase you need to make corrections, revisions or updates, use this
    same link and make the changes yourself.
  • Once you have completed your on-line application, you only have six (6) months to complete the application process before your application expires.
  • Applicants should print out the required forms associated with your initial application. Each applicants will have a unique ID number that is associated with your application that will be
    preprinted on these forms. All required documentation, forms and fees need to be returned to us via U.S. Mail to: Attn: PRSS, P.O. Box 53277, Oklahoma City, OK  73152-3277

PRSS Certification Renewal Information - 

 Replacement Certificates

  • Applicants may send a request by email to Ramona Webster or Ronda Trumbly requesting a replacement Certificate.  Ramona or Ronda will email an electronic copy of your certificate that you can print out.
  • If you need an original certificate on brown/green paper, you must indicate so in writing to PRSS along with the replacement fee ($5.00) to Attn: PRSS, P.O. Box 53277,
    Oklahoma City, OK  73152-3277 via U.S. Mail.
  • There is a required replacement fee of $5.00 for an "original" certificate that is lost, damaged or in need of revision.  This fee can be paid by check or money order.
