State Health Department Announces Long Term Care Survey Results Now Available Online 

Information on the quality of care provided in long-term care facilities in Oklahoma is now available online at no cost, the Oklahoma State Department of Health (OSDH) announced today.

Consumers can visit the OSDH Web site to view or print inspection surveys and other public records of adult day care centers, assisted living centers, intermediate care facilities for persons with mental retardation, nursing facilities and residential care homes.

The OSDH site allows consumers to see reports on these facilities even before they are posted on the national Nursing Home Compare Web site, which provides details on Medicare and Medicaid-certified nursing homes in the country.

“Consumers will be able to easily access survey results and public file information, and do their research in a more timely manner,” said Dorya Huser, chief of the OSDH Long Term Care Service.

Huser said the new Web site provides users with detailed instructions in locating facilities. Consumers can search for information by name of the facility, city, county, or zip code and assess the facility’s quality of care by reviewing the health and physical plant inspection results. 

“Before choosing a facility, consumers should review the survey results, visit the facility being considered, talk to the health care provider caring for the patient, and call the Long Term Care Ombudsman at the Department of Human Services for more information,” Huser said.

Candidate Filing Packet for School Elections Available Online

Candidates for members of the Board of Education can find the complete school election candidate filing packet online at the State Election Board website. The packet contains the required Declaration of Candidacy form as well as requirements for candidates, instructions for filing and other information.

Filing for members of the Board of Education opens at 8 AM, December 6, and closes at 5 PM, December 8. Other important dates include:

  • Annual School Election — February 8, 2011
  • Annual School Runoff Election — April 5, 2011

Consumer Use Tax Payments for Individuals Now Available Online

What is Consumer Use Tax?

Consumer Use Tax is due when you have made a purchase from out of state and you were not charged Oklahoma Sales tax on the purchase. Out of state vendors are not required by law to collect Oklahoma tax if they have no physical presence in Oklahoma, but tax on the sale is still due. This system was created to allow individuals to pay tax directly to the Oklahoma Tax Commission on items purchased from out of state.

The amount of Consumer Use Tax due depends on where the item is being delivered and used in Oklahoma. All items are subject to the 4.5% State Use Tax rate. Then additional tax may be due for the County and/or the City where the item is being used. This program will direct you to the Oklahoma Tax Commission Rate Locator program to determine the proper tax rate and City/County codes for your location. Once you have made note of this information, you will enter it in the proper fields and calculate the tax. Your payment will made by electronic funds transfer from a checking or savings account.

All OK.gov services will be unavailable on Saturday, December 4th from approximately 9am to 5pm due to the relocation of OK.gov’s servers to the new Office of State Finance data center.
